At Transitions LifeCare, we strive to be the one-stop solution for patients, caregivers, and healthcare providers by providing healthcare resourcing, care setting transitions, patient empowerment, palliative care for severe illnesses, and much more.

As we develop new initiatives and programs, our primary focus will always be the elderly population within our Tulsa communities and coverage areas. We aim to assist this demographic with navigating the complex healthcare systems, ensuring patients and their families achieve their ideal outcomes.

TLC is family owned with more than 75 years of combined executive-level healthcare experience. We are dedicated to meeting the needs of our city and surrounding communities.

Our history of providing high-quality healthcare services in a number of settings is an achievement we are proud of and speaks to our commitment to the community we serve. Our experience in these healthcare settings serves to improve our understanding of the many resources available as well as the need for communication between those resources.

Our hospice services are easy to identify with the name Transitions LifeCare Hospice. We are licensed in Oklahoma as a provider of hospice services and have provided these services as a contracted provider with Medicare since 2006. Our Medical Director has maintained his Hospice and Palliative Medicine credentialing since 1999, as a specialist in this field of Medicine. Our presence in this community has always been premised on providing care that exceeds the expectations of the patients and families we serve. This premise is operationalized by hiring clinical professionals who want to work for a company that has high standards of care, teaches and develops their staff to deliver on that high standard of care, and holds themselves and their staff accountable.
